How to Start an NEMT Business: A Practical Guide for New Providers

Starting a non-emergency medical transportation (NEMT) business can be a strong opportunity, but it’s not a simple “buy a van and start driving” operation. NEMT is a regulated industry that involves compliance, licensing, insurance, training, operational planning, and long-term infrastructure.


There are many different paths to starting an NEMT company. Some providers start with private pay, some focus on facility contracts, some pursue broker relationships early, and others build hybrid models. What doesn’t change—regardless of the path—is the level of research required to do it correctly. Understanding regulations, insurance requirements, market demand, operational systems, and financial structure is not optional. This research phase is what helps providers avoid costly mistakes and truly understand the business before stepping into it.


Many new providers enter the space without fully understanding the systems required to operate sustainably. The result is often delays, unexpected costs, compliance issues, and operational setbacks that slow growth before it even starts.

Building an NEMT company the right way means treating it like a real business from day one—not a side hustle.


The foundation starts with business structure and compliance. You’ll need to properly form your business entity, obtain your EIN, register with your state, and secure any required local and state transportation licenses or permits. Many cities and counties also have additional transportation regulations, so local compliance matters just as much as state-level requirements.


Insurance is one of the biggest hurdles for new providers. NEMT insurance is specialized and not every broker or carrier understands the industry. Coverage typically requires commercial auto, general liability, professional liability, and sometimes additional policies depending on your service model. Underwriting often includes interviews, documentation reviews, and operational vetting, and the process can take weeks—not days.


Vehicle selection is another critical decision. Your fleet should match your service model, whether that’s ambulatory, wheelchair, stretcher, bariatric, or mixed services. Modifications, lifts, securement systems, and ADA compliance are all part of proper setup, not optional upgrades.


Training and certification are non-negotiable. Drivers and operators are required to have proper safety training, including CTAA PASS wheelchair securement certification. This protects passengers, drivers, and the business while ensuring compliance with broker, facility, and regulatory expectations. Operational systems are what separate sustainable providers from short-lived startups. Revenue planning also matters. Many startups focus only on broker contracts, but broker work is volume-based, lower-margin transportation. Sustainable providers build a mix of private pay, facility contracts, discharge partnerships, and insurance-based transportation to create financial stability.


Starting an NEMT business is not about speed—it’s about structure. The providers who last are the ones who build correctly from the beginning, invest in compliance, and create strong operational foundations before scaling.
