When people first start looking into the Non-Emergency Medical Transportation (NEMT) business, one of the first questions they ask is how many vehicles they need to get started. The honest answer is that it really depends, and there isn’t one number that works for everyone.
Instead of focusing only on a vehicle count, many new providers find it helpful to think about a few basic considerations.
Demand Is Only Part of the Picture
It’s easy to assume that more vehicles mean more trips and more revenue. In reality, having demand doesn’t always mean you’re ready to support it. Scheduling, driver availability, and daily coordination all play a role in whether additional vehicles are actually useful.
Some Costs Exist No Matter What
Many startup costs are the same whether you have one vehicle or several. Insurance, licensing, compliance requirements, and administrative time often apply across the board. Because of this, adding vehicles doesn’t always reduce costs the way new owners expect.

Each additional vehicle usually means more drivers, more coordination, and backup coverage when things change. Without enough support in place, growing too quickly can feel overwhelming rather than helpful.

Some NEMT companies choose to start small and grow over time, while others expand based on contracts or partnerships. Decisions around fleet size are often influenced by comfort level, insurance costs, and cash flow timing.
Final Thought
There is no “right” number of vehicles to start an NEMT company. For many people, the better question is how their vehicles, drivers, insurance, and daily operations fit together at the stage they’re in.
